Appointments
Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and we appreciate your cooperation.
Insurance
Our office is committed to helping you receive the maximum benefit possible from your insurance. We will provide you with an estimate of insurance coverage prior to beginning any treatment. Please understand, however, that this is only an estimate. Because insurance policies vary considerably, and because many contracts contain exclusions and provisions we cannot anticipate, it is in your best interest to be familiar with your own plan. We offer no guarantee of your insurance coverage.
Your estimated "patient portion" of the fee for your treatment is due at the time of service. As a courtesy, we will bill your insurance company for the remainder. If you have any questions, our courteous staff is always available to answer them.
